I needed an ad to place in the yearbook to promote my photo, but I couldn’t find any full page ads. This is the closest thing I could find and it worked well. Layers are nicely grouped and in their own folder- I always appreciate it! I made some changes because I have more verticals than horizontals, but this is a good base. A model release form is a contract between a model and a photographer, often used in fashion photography or corporate photography. Model releases allow photographers to use images for promotional purposes or sell images commercially.
The short answer is no, they are very different. As mentioned above, model releases are signed by the talent so photographers can license or use the image. On the other hand, a photo release form is a document signed by the artist and given to the client to print a copy of the work (ie portraits, headshots or weddings, etc.).

For example, let’s say you’re drawing a head for an actor. They’ll sign a model release and you’ll give them a photo release to replicate.
Fast forward a few years and they want to sell photos on their website. But they have to pay you a license fee to do that, otherwise they’re infringing your copyright.
In the United States, photographing individuals in public is a protected right for photographers because no one is guaranteed privacy in a public place. An exception is that photographers cannot sell the images commercially without releasing the model. So if you shoot amazing street photos with a simple subject, then get a signed release.
