Psychotherapy Progress Note – In fact, almost everyone needs therapy at some point in life. Therapy isn’t just for people who struggle with depression, anxiety, and other mental illnesses. Even average looking people really need therapy. Therapy sessions are helpful in allowing patients to release their pregnant feelings and thoughts. Almost everyone likes to be heard. If you feel that you need professional help to listen and help you through difficult times, you should seek therapy.
At each therapy session, you can see your progress using notes provided by your therapist. The note contains the identity of the patient and the analysis of the story given by the patient to the therapist. There are different types of therapy progress notes. The most important part of the progression note is that the therapist must be careful when diagnosing and providing this type of treatment to patients. Some patients who cannot explain with stories may lead the therapist to misdiagnosis. Also, use simple language that patients can understand when reading therapy progress notes.
Psychotherapy Progress Note
Because therapy is one way for patients to trust others when they show their true selves, it is important to maintain their privacy. Only therapists and patients can know the progress of the therapy. If others find out, it can worsen the patient’s condition by burdening the mind and emotions with trust issues. However, if it is really necessary, for example, the patient is a person under investigation for committing a crime, which allows someone other than the therapist to know the progress of the therapy.
Writing Soap Notes, Step By Step: Examples + Templates
The existence of the therapeutic document is one of the authentic proofs regarding the personality of the patient. If the patient is a very important person and his condition really needs to be kept secret, then the session can be connected to therapy without leaving a comment. In addition to notes, there may be other alternatives that only the therapist and the patient know about. Sometimes the results of therapy notes are misused by people who mean bad things to these patients. Therefore, care must be taken when choosing a therapist so that the process and therapy sessions performed are not in vain. Instead of leaving a note, you can make a recorded version of the audio and practice the patient’s activities outside of therapy. Thus, this development can be seen from performance through practice.

The therapy progress note informs the prescribing physician of the patient’s current condition towards his or her rehabilitation goals. This type of note can also replace daily notes as it follows the standard SOAP formula for daily documentation.

When writing a progress note, be aware that the doctor may have less than a minute to review the notes. Be clear and to the point while highlighting the most important findings in your review.
It can sometimes be useful to place an asterisk or asterisk next to an important finding if you feel it will go unnoticed in the rest of the process note.
Provide a brief statement of how the patient feels about their progress since starting PT (subjective experience of therapy).
The patient reports 3/10 pain in his shoulder, but feels overall stronger since starting PT. Now you can reach head level without a sharp pain in your shoulders.
Therapy Note Templates
The progress note also includes a summary of treatments and the number of visits completed. This gives the doctor an idea of how many visits/cancellations their patient has had and what type of treatments they have provided.
Treatments may include: therapeutic exercise, manual therapy, neuromuscular retraining, balance/gait training, iASTM, E-Stim, ultrasound, etc. I’ve included a line from this summary in the progress note sample template.
It is important to include relevant special tests in the objective report – key word: “relevant”. Do not include every possible special test; this makes it difficult for the physician to detect relevant findings. Include only the most relevant tests and indicate whether this is a positive (+) or negative (-) result.
Recording a previous ROM is a quick way to document progress or regression. It is also useful to present the data in a grid format, especially for ROM and strength measurements. You will see that my progress note template includes this format.
Progress Notes And Psychotherapy Notes
In this section of your progress note, the doctor wants to read your general thoughts about the patient’s progress toward goals and how you expect treatment to progress.
The patient met all short-term goals and is progressing well toward long-term goals. He doesn’t have full ROM in shoulder flexion, but that should improve as we continue to strengthen. In the next phase we focus on eccentric strengthening as well as motor control for better scapular mechanics. Continue the therapy 2x per week for 3 weeks in order to achieve the goals
Some therapists combine the Assessment and Plan sections into one progress note. However, you may notice that I have included a separate line for the Plan in my sample progress note (download here).
Shoulder strengthening continues to evolve, including stabilization techniques and PNF hand resistance. It is recommended to continue twice a week for 3 weeks.
