This patient went to an orthopedic specialist to discuss hip replacement. In the note, the doctor describes the patient’s visit and symptoms. The doctor also outlines several further steps for the patient, including follow-up visits. The patient used the note to remind himself of the appointments he needed to make. He also shared the note with a family member who helps look after him and wrote the note on a file at home in case he wanted a second opinion.
This patient has weekly treatment visits with a social worker to discuss feelings of depression and anxiety that have worsened since the recent diagnosis of Parkinson’s disease. In the note, the therapist describes the symptoms of the patient’s physical and mental health and outlines some of the coping strategies discussed during the therapy session. This patient was relieved to read that her therapist understood her situation. Parkinson’s affects his memory, so being able to view his notes helps him remember what he needs to do between visits. In addition, the patient can print a note and bring it to the doctor who is prescribing him, so that both members of his medical team can be informed of his care.
This patient has type 1 diabetes and at least two other serious medical conditions. He takes a few medications and uses his notes regularly to manage his health. She remembered a time when the dosage of the drug had seemed wrong, when she had gone to the pharmacy to get it. He looked at his notes to recall what his doctor said and was able to adjust the dose. In addition, the endocrinologist treating her diabetes is not in the same hospital where she receives the rest of her care, but can print notes and share them. It is important to him that his entire healthcare team has access to the same information. The notes helped her feel like an expert in her circumstances and says that despite her complicated medical life, she feels healthy.

It didn’t work well for my patient flow, and I didn’t really use it. And “NPO” was spelled “MPO”.
