Temporary Custody Form

Temporary Custody Form – The Washington Guardian Minor Power of Attorney form is a legal document that parents or guardians use to place their child in the care of a trusted friend or relative when they are ill or long-term absent. It should be noted that all those responsible for the child’s care have signed this document. Therefore, if two parents have custody of a child, both parents must sign this form to authorize the representation to exercise custody of the child. Parents will always retain control over the child and can determine the extent of the guardian’s authority and/or the specific time period they work. If more space is needed for additional information, continue on the sheet that was added and attach the document. This document can be withdrawn in writing at any time by the parents/guardians. Such revocation must be delivered to the person whose custody is being revoked.

For Medical Emergencies Only – For use only, as defined in RCW 11.94.010(4), to assume responsibility for emergency medical care decisions on behalf of a minor.
