The following logic model is an evaluation model with plans to complete TB treatment using incentives and enablers. Other methods can also be used to construct logical models. Performance and job evaluation programs provide additional examples of simple and complex arguments and checklists for their development.

A logic model is a great way to document the programming process, from logic and intended purpose (theory of change) to inputs, outputs, and results. By using a logical model template for your programs, you can think about what you really want to put in and get out of your program. When your project ends with an outcome, you can use a logic model to evaluate it – did the inputs you put into your design have the expected outcome? Did those outputs develop into long-term engagement?

Alternatively, you can find a blank template in spreadsheet format and other useful templates for project plans and estimates that you can open through Google Docs and copy by selecting “Create a Template” from the File menu. Then you’ll have your own template to modify and use.

A logic model is a detailed graphic representation of how inputs and actions will lead to outputs and short, medium and long term effects. They are useful for planning your journey to your destination. Logical models are divided into four broad types – pipelines, output chains (or output hierarchies), matrices and pragmatic descriptions (Funnell and Rogers, 2011).
This logic model was developed by the INWPCP not-for-profit partnership group in collaboration with the University of Melbourne. It outlines the company’s focus on individual (blue), community (orange) and organizational (purple) environments. The long-term outcomes align with the variable (VicHealth, 2009) on preventing violence against women (VicHealth, 2017) and the changing story (VicHealth, 2015) calculated in Vic Health frameworks.
